The person named Neni who answered this question has it goin' on. That's what flaky means; however it could also be a variation of what she described.

A flaky person can also be an alcoholic, or someone who has a drug problem (unfortunately society doesn't have any shortage of these), a crook, liar, con artist, etc.

It's very common American slang. Someone who is flaky is unreliable, inconsistent, can't be counted on. They're not "bad" or shady people, they just, for example, tend to be late all the time, unprepared, cancel or don't show up at the last minute. You get the idea.
If someone "flakes out" on you it means they stood you up or canceled on you at the last minute.
