Dr. Rice has a doctorate. She was a professor.
Dr. Ahmadinijad has a Ph.D. in transportation engineering.
Robert Mugabe has a number of honorary degrees, but could also be called Doctor Mugabe.

To answer your question, many people do not like either Dr. Ahmadinijad or Dr. Mugabe, so I imagine they take out their annoyance with them by failing to use the proper title.

In my opinion, one should always use the proper title, as it is courteous, whether or not one agrees with the person. Failure to use another person's title does not hurt the other person, but reflects badly on one's own manners.
