Two things: First, study so that you know the material inside out. Include answering test questions in your studying. Second, mentally rehearse the exam itself. To do this, sit or lie down in a setting with no distractions of any kind. Visualize yourself getting up on the morning of the exam, doing all the things you usually do in the morning, going to school, taking any classes you may have before the exam, going into the exam room, looking through the exam paper, answering all the easy questions first, then the medium hard questions, and finally the hardest questions, remembering that you know all the material and will get a good mark. Do this at least five times before the day of the exam. Then go in and ace the exam - you've already taken it five times!
This is a proven method to reduce all test anxiety and free your mind to focus on the questions and your knowledge. It can be used at almost any school level, from intermediate grades all the way through university or college.

this worked for me... (i knew that somebody advised me to do this but i cannot recall who...)
when i was preparing for my engineering licensure exams, my room's walls were almost covered by different review materials, formulas and other important things that i needed to memorize...
these review materials were divided into subjects...
it became so that when i need to recall a particular thing, i only needed to picture the wall where the subject was locted and i have no trouble recalling what i needed...
i suspect that it became retained in my memory because i had the review materials pasted on my wall for a long time that i nearly had total recall then...
i passed my licensure exams on the first try!

Study, eat something before exam, clear thoughts, plenty of rest. But most important to eat something before an exam.
