There are several major arteries in the groin region. A well aimed kick could crush them, causing internal bleeding and eventually death.

Possibly >If the kick was hard enough to do create a lot of blood clots it's possible that one of those blood clots could get loose and lodge in a place that could be fatal.
On the other side of the coin , it's also possible that the person doing the kicking could wind up getiing there azz kicked -badly
because contrary to popular belief a hard kick in the balls may not bring a man to his knees It might just get him mad.

It is possible but unlikely. If you hit the person hard enough and in the right spot they could go into shock and die.
Most of the time though they if there is any serious damage it might be a ruptured testicle. The testicles can take pretty good blows though. If they are free to move they can usually take an impact without and serious damage. If they are held in a fixed place where they can't move then they might rupture.
Some guys even like to be kicked there for fun, but most don't. I enjoy it and think it is fun and funny most of the time.

Probably it is possible, for instance if a blood vessel was ruptured or they became infected later or something. But mostly it just hurts a lot.
