annovulation, most likely.
it can be caused by a very slight imbalance (not even anything major lacking or in excess, just not quite in the right proportions).
I actually have had to be on bc pills most of my life to force my cycles. i went 2 1/2 yrs in my late teens without any sign of a cycle. any time i'm not on the pill i have the same problem. the doctors ran a TON of tests, couldn't figure out what was causing it, EVERYTHING was normal. even the specialists couldn't figure it out, they were about to put me on meds to force it again. apparently i just didn't release an egg very often.
(i obviously did eventually, as i'm pregnant!)
so it might not be anything serious, but she should definitely go to a doctor & make sure.

maybe a health condition or lack of development in the female organs. maybe she has a csyst on the ovaries, or maybe her female organs didn't develop properly. Could also be a hormone inbalance. Maybe her estrogen levels rarely reach high enough to have a period. Has she consulted with a physician? It could be lots of things. but if she hasn't seen a doctor then she really needs too.

there's many possibilities, but the major underlying cause is that she doesn't ovulate. Ovulation starts the hormonal process of menstruation. If she never or rarely ovulates she will have none to very little periods.
Is she underweight? low body fat may be the reason
Is she overweight and/or diabetic? she may have PCOS (poly cystic ovaries)
There are other reasons but these are the main ones.

there are some methods of birthcontrol that are said to reduce the number of periods a woman gets but i don't know what could cause it to be that few she should really see a docter and how do you know how many periods she's had during those entire 15 years? maybe she's playing with you?

If she's anorexic, she is not likely to menstruate and if thats not the case, then something must be wrong. Not going to the Dr. is really, really dumb. She obviously doesn't care what happens to her body. Or she might already know the reason and she just doesn't want to tell you too.

Extreme weight loss could affect that (i.e. if she were anorexic or something) but it could be something having to do with her reproductive system. Medications or pills or any number of things could affect this. A doctor would know best, but if she has seen one and the situation is under control with them (with whatever they have suggested) then it's probably nothing to worry about.

Health issues can do that. Many runners, gymnasts, etc. do not have a period or seldom have one. Or those with eating disorders, OCD, or similar things that keeps their weight extremely low. If the body has not enough fat or nutrition, it will stop ovulating.

Sounds like she has some medical problem that needs to be checked out by a doctor.

Possibly PCOS. It's a hormonal imbalance. Too much testosterone or maybe not enough estrogen.

She wouldn't get her period if she was on the pill and didn't take the placebos, if she is overly athletic, or has an eating disorder
