My friend sent her parents a frame for Christmas. It was one with 3 places for pics. On either side she put pics of her 2 kids and in the middle an ultrasound pic to tell them that she was expecting again. It was a really nice frame and her parents loved it.

Make/buy a snowman family that has one extra family member in it and say hey look there is one for everyone in the family.
Buy one of the family tree photo frames and put a picture of each grandbaby they currently have in it, and then shrink down a picture of the first ultrasound as one for the baby.

If you get an ultrasound before, print out the picture and put it in a large gift box, along with maybe a grandparent-type ornament? Wrap up the box as usual!

My sister-in-law made a jello type dessert and hid a pacifier in it. We she dished it out, someone ended up with on their plate. It was cute, and everyone got all excited.

take them to a resaturant and have them open a gift that will have t shits in it saying grand ma on one and grand pa on the other
