Forgive, it is best, but move on, that is even more important. Once someone shows you that they can not be trusted, that is it, they can not be, and they are not mature enough to know right from wrong. Good bye.

Forgiving doesn't necessarily mean staying with him. Forgiveness is for yourself. But you also don't want to be stupid. If you continue to stay with him after making these "BIG" mistakes. Whether subconscious or not, he will get the message that you are willing to accept disrespect and he will continue this sort of behavior.
...Seems to me it's time to kick him to the curb.

That depends on the mistake, how much you discussed his first mistake, and how much you two like each other and get along in general. Personally, I'm a really easygoing person, so I'd just forgive him.

He didn't make a "mistake". He did it once and you let him get away with it. So he did it again. He's playing you like a cheap dime-store fiddle. Trust me, I've seen guys do it time and again, and heard them brag about it. Might have even done it once or twice myself.
