Well, if her name is different from the mother's name on your birth certificate, that is a problem, a serious problem. you'll need to show somehow that the woman you've petitioned is, in fact, the same woman on your birth certificate. Not just show, but prove. Did she change her name somehow? Show how that happened. If you can't do that, you'll need to prove she is your biological mother in some other manner. Sounds like a good time to do a DNA test for the two of you.

Your mother must execute an affidavit narrating the loss of the marriage certificate and the change of names to prove her identification and have it notarized in order that it will be a public document.

either your mother was married or not, if her last name has changed? than, you have to give the immigration the reason why or how ?
is she single or not?
why has her last name change? she is your mother? you should know .......
you will not be able to get her trough immigration without proof .......
