Mail her a W-9 (request for taxpayer identification) to her old address, it will get forwarded to her new address.

Did she babysit in your home, or hers? If in yours you are going to end up responsible for in home employment taxes. Watch this, it may be more expensive to claim it.

Go to www.irs.gov and search on dependent child care expenses, they will have a lot of info you need.

Good luck

last thought, for around $50 you can go to www.ussearch.com and you probably will be able to locate her and then you can send her the W-9 certified, return receipt requested.

See IRS Pub 503 for instructions on how to claim the credit if you don't have the provider's SSN. You must make a due dilligence effort to get the SSN and document what you did. The IRS will then allow you to take the credit without the provider's SSN.

It is not clear what kind of credit you are expecting from the IRS. I would think you may in fact need to pay the SS tax as her employer.

If she was in fact an independent contractor then you really don't need to worry about her SS number.
