I've read of some military people receiving these for meritorious service but not a lot of explanation about where they're from and how they're made, designed, etc.

Most "Coins of Excellence" are made through a minting company. These coins are given for meritorious service or an action that doesn't quite meet expectations for military honors. It's more than a pat on the back but less than a medal.

Drill Sargeants get coins of excellence too, after graduating from drill school, and I've seen some others for NCO's Non Commissioned Officers as well.

Navy calls them Challenge coins or Command Coins. anyone can have them made up, most Flag Officers have personal ones, and most CO's have them printed up to hand out as pats on the back to deserving servicemembers.
