Pretty straightforward.
Just grab a cup of coffee with your manager. And just lay straight the facts as to why you feel uncomfortable with your current job.
Just think through the various excuses and carrots that you are likely to be dished out when you inform of your decision. And being well prepared to handle it should make it smoother.
And more importantly..Blame the job and not the Manager :-)

Give your employer two weeks notice. You can give your reason if you want to or simply cite personal reasons.
Make sure you are polite and you can ask them for a reference if you are on good terms.
It's never a good idea to burn bridges when you are leaving a job.
