Ultram (Tramadol) is used to relieve moderate to moderately severe pain. It also may be used to treat pain caused by surgery and chronic conditions such as cancer or joint pain. Ultram works by decreasing the brain's perception and response to pain. It also reduces the size or magnitude of the pain signal passed from one nerve to another.
Good luck!

You need to provide a little more info to get a really accurate answer.
What is causing your back pain? Have you been evaluated by a pain clinic and /or a Physical Therapist as well as your personal Doc?
How long have you been on the same pain med, tolerance can be built up and switching to a different same category contolled substance can be effective.
Have you had any alternative pain relief such as biofeedback, accupuncture, or distraction therapy in conjunction with pain meds???
Are you over weight? A large stomach puts extra stress on our back.
Are you able to excersise at all, yoga stretching and meditation can usually be done by even the bed ridden.
As to the "strongest" pain medication available, I realize you are in pain, so am I, but do you really think your pain level rates up with some of the Cancers, or a recent train wreck/car accident or multiple other pain problems that Morphine or Demerol are used for.....
Have your Doc's considered muscle relaxers or anti anxiety or anti depressants or anti seizure or many other meds, that have the ability to interact with and make your pain med work more effectively.
I believe your pain is real and the question is one we all ask when the pain is out of control and stress or "something" has prevented us from getting adequate sleep. I hope you will use this info to communicate with your health care proffesionals.
Good Luck usually chronic pain people do find a good combination of the above suggestions does get us some relief some of the time.

MS-Contin (Morphine Sulphate) is pretty strong!! It is a long-acting morphine. When I fractured my back I was on this.
It's nasty unless you like having extreme constipation, nausea and your personality changes (and you don't even realise it)
Duragesic Patches are also used for the treatment of long-term pain and they are pretty strong. They give this for Cancer patients.
I'm a long-term pain sufferer. Do yourself a favour and don't become dependant on the strong narcotics.
Ultram or Ultram-ER are really a great medication--it is indicated for moderate to moderately severe pain. The generic is Tramadol. It is not a narcotic, but seems to work on the same brain receptors.
I also go for massages, chiropractics, acupuncture and nerve blocks. I'm under the care of a Pain Management Centre and it took about 3 years to finally find the Ultram. It does not cause the nasty side effects that Narcotics do and it really gets rid of the pain! Since it is not a controlled drug, doctors are pretty agreeable to prescribe this.
