Now that I think about, madhavan n might be right. I think it means that if you ask someone how they are in Britain, they'll say they're "okay." But if you ask them in any other country, they'll say something else (all right, good, fine, etc.). But it's not really funny when explained that way. Perhaps there's another explanation?
