Yes, you do. They should have provided you with a TIN number upon being hired from the USA. (Tax Identification Number) It is on their work visa.

It depends upon the visa to some extent, however in most cases you do have to withhold taxes exactly as if they were a US citizen.

If they are on a H1 or L1 Visa they should have a Social Security Number. You would withhold all of the same taxes that you would for a US Citizen.

Yes, unless either (a) a tax treaty between the U.S. and their home country says otherwise, or (b) you are a church or other organization that is not required to withhold tax from anyone.

Yes, you pay taxes in any country you are working in. Even student visa pay taxes if they are working, moreover, you are on a work visa, you should be paying. You are enjoying the benefits of the taxes while you are here.

Yes. Any income made in the US is subject to us taxes. I was in the US for many years on a VISA and I had to pay taxes.
