When I had hives I was given various antihistimines and steroid creams, not which were very effective, but suddenly after about 7 years, the condition just vanished of its own accord. I agree with one of the answers that she should be referred to a skin specialists for some allergy tests, although this is time consuming and not always one specific cause. I found that mixing bicarbonate of soda with warm water into a paste and putting it on the worst affected areas brought immediate relief, although very messy, but it sounds like your girlfriend has it very severe, so would suggest you seek more specific advice. Good luck.

Have you ruled out temperature allergies, like being allergic to hot or cold? My brother is allergic to the cold and breaks out in bad hives all over his body for hours after being exposed to cold weather, water, or food. This is all I can think of at the moment...Sounds really terrible, poor little girl. :( I really hope you all find out what is wrong and find a way to treat her!

Rather than going to an E.R., I'd take her to a dermatologist or an allergy specialist. Until then, try Bendryl (read the bottle for the maximum dose allowed) to relieve the symptoms.
