pick up their kids after preschool is out. So not just any one can pick up the kids from school!

If you're having trouble finding one online, try making one yourself. It doesn't have to say too much only something along the lines of "I _______ give permission to the following person(s) _______ to pick up _______ from school if I am unable to" etc." and then sign it and get the teacher to sign it.

Most schools I know of use a simple table with the students name pre-printed and in and out over the next 4 columns one set say "signature - time in" the next set says "signature - time out". It is up to the adult attending the students to confirm the identity of the signer and assure the child is released to the correct party. Good Luck!

You need to get the form from the preschool your child goes to. They probably won't take anything their own lawyer hasn't approved.
