I think they're seeing that your pupil functions properly and also somehow looking niside your eye. I agree, it's torture!

When I was doing my professional exams, 28 years ago, I heard somebody failing theirs in the next cubicle.

"What did you notice when you checked the pupil reactions?"
"I didn't check the pupil reactions."
"In that case we have no further questions."

Pupil reaction anomalies are relatively rare. I'd probably do it on 3-400 people before I got an odd reaction. But for that person it would be significant, and could be very important.

Horner's syndrome, perhaps.
Hippus, a few times.
I've found a couple of ciliary tumours with pupil reactions as the first clue.
More often it's just a pointer that there's *something* unusual going on, no more specific than that.

But it's certainly not done just to annoy. And it should be done.

That bright light they use is called a retinascope. It has nothing to do with checking pupil reaction. It is to see inside the eye to check for any anomalies, such as the beginning of cataracts on the lens of the eye. They can also see if the retina looks healthy , way at the back of the eye.

looking at a bright light won't make you go blind. Looking directly at the sun for a long period of time isn't great for your eyes, and you can go blind if you look at a solar eclipse, but just looking at a bright light isn't bad for your eyes.
