I have very severe adult ADD. I have been on and off medication all my life for it.

Like ritilin, adderal, etc....

But I hate the way it makes me feel.

But sometimes when I really need someting to help me focus, I find that coffee helps a little - and gives a similar effect.

Why?

I don't think the answer is known on a molecular level. The discovery was somewhat accidental. It was noted that barbiturates such as phenobarbital made persons with ADD hyperexcitable. Someone reasoned that if phenobarbital had a paradoxical effect, what would happen if we gave them a stimulant? And lo, it often worked. Certain stimulants have a paradoxical effect on persons with ADD. They are not sedating, but rather help the affected person to sustain a focus of attention.

Not every person with ADD tolerates medication. Please discuss the issue with your doctor. There should be a few alternatives that may help you. Good luck.
