if two people who are closely related get married and have children together, more than likely the child could come out deformed or mentally ill.

The whole idea was that two closely related people BOTH could have an inherited predisposition to a specific disease that might run in their family. That makes it more likely the child COULD get that disease rather than if only one parent had that disposition as would be the case if the parents were from different families. If in-breeding is done consistently generation after generation, there can be physical effects, but that doesn't happen anymore. If first cousins have children, the children will be normal.
