Well I saw a loaded semi hit the drivers side of a Dodge Dynasty

The semi was doing 55-60 MPH.

The drivers head was in the passengers lap - both were dead on impact

There are a lot of variables when it comes to motor vehicle collions. However, anything that could make a person bleed out before they are transported to the hospital. As EMS workers our concern when doing a rapid trauma assessment is anything thing injured between the nose to the knees.

nose: any blows that would cause blood/spinal fluid/or brain matter to be seen
mouth: any force that cause person to loose teeth and possibly causing accidental inhalation leading to airway obstruction.
neck: any force causing cervical spine to be fractured C1-C7 housing nerves that control breathing and cardiac function
chest: any force severe enough damage the aorta, heart, and lungs
abdomen: the major concern here are the solid organs (spleen, liver, kidney, pancreas) reason they can hold a lot of blood at any given time causing sever loss of blood.
pelvis: this is a strong bone to brake. EXTREME force is needed to brake or crush. There are major blood vessels here and damage to these vessels will lead to death in minutes if unstable.
femurs: same as with pelvis the force will be extreme there are also major blood vessels and if the fracture is unstable blood lost can lead to death.

Receiving an one or more injuries in these areas can cause a fatality on scene.

Seat belts should be warn snug not loose. Pregnant occupants should wear lap belts under the belly. NEVER PUT KIDS ON OTHER PASSENGERS LAPS!!!!!!!!!!!! THEY SHOULD BE IN CAR SEATS OR BOOSTERS IF 80LBS OR LESS!!!!!!! ANY CHILD UNDER 5 FT SHOULD NOT SIT IN FRONT SEAT WITH AN AIRBAG!

Just wear the stupid seat belt for the love of everything holy!

I work as a firefighter/paramedic, so I have experience in 'seeing' it first hand. First of all. all your organs are "on belay" in your body. Meaning. they are hanging by a thin layer of tissue. Therefore, three things happen in a collision. Car hits objects absorbs energy, body moves in car seatbelt pretensioners and load limiters dissipate more energy then your internal organs are still moving while your body has stopped and they impact the internal structures around. IE heart impacts sternum and ribs. This is the internal damage people talk about. In a severe enough crash, the forces can be great enough that your body is fine. No cuts or seen injury but internal bleeding can occur or severe bruising causing quick deaths. Bottom line speed does kill and bigger vehicles win over small vehicles and nothing beats a semi.

stay safe

Head and internal mostly when it's fatal, which mean someone died.
