since i always see patients with dilated pupls in the hospital and they indicate certain diseases, i wonder what kind of disease that very constricted pupils is manifested.

It's hard to interpret constricted pupils by themselves. You have to interpret the pupils in conjunction with the other stuff that's going on with the patient. That being said, some reasons for constricted pupils are,

1. Normal eyes in bright light - pupils normally constrict with light so if you look into people's eyes outside, you'll find that they're smaller than what you're used to seeing indoors.
2. Opiates - heroin, morphine, Vicodin, codeine, etc. can cause constricted pupils. The more drug, the more constricted.
3. Bleeding in the brain - a bleed in a specific spot in the brain can cause constricted pupils. This spot tends to be a very bad spot to have bleeding so the patient will manifest other signs in addition to the pupils.
4. Chemicals in the eye - some eyedrop medications cause the pupils to constrict and some medications and chemicals, if accidentally applied to the eye will do the same thing.

There are probably many other causes - maybe someone else can suggest some?

Interestingly, about 10% of the population will have one pupil just a bit smaller than the other. It's usually fairly subtle and you have to look very carefully to pick up the difference but it is there.
