His only symptoms are ear fluid and terrible pain. He has seen his family doctor several times and the ear ache heals and within a month or two it is back.

He could have an inner ear infection, there are viruses that cause these symptoms and they can be treated with anti biotics. He may also have wax build up even if you can't see it, the doctor can with his little light. He needs to go see a doctor.

Suffering from chronic allergies, that may not even be severe, can cause fluid to build up in the inner ear which leads to infection. Congestion is not isolated in the nose only. Your husband might do well to use an OTC med such as Clariten D (the one behind the pharmacy counter) as a preventative or he can ask his doctor to prescribe a decongestant-antihistamine.
He could also sleep with an extra pillow or wedge the upper portion of your mattress so that his head is on a slight incline rather than flat.

Maybe too much bacteria in the water. Try bottled water.
