I teach a module about this in a class I teach on life priorities. Here are my tried and true suggestions:
1. Give up on achieving perfection and be content with excellence, as perfection is a chief latent cause of procrastination.
2. Set and work on a schedule for your day, week, month, year. Do the most difficult (and usually most important) stuff during the time of the day when you have the most energy.
3. Form an accountability circle as most of what I have gotten done in life if because I was ashamed not to get it done because others were holding me, at least passively accountable, i.e., parents, wife, children, boss, professors, friends, students, etc..
4. Read and live by 1) 7 Habits of Highly Effective People - Covey, 2) Getting Things Done - Allen, 3) The Power of Full Engagement - Loehr, and 4) Think & Grow Rich - Hill
Be proactive (versus procastinative). :-)

A wise man once told me: If you want to be the happy and content you need to confront things head on! This is true as once you have done them, you feel so much better for getting of your **** and bothering to do it! And it's done :D
