How come you pay an extra 25 cents to get something put on your hamburger but they don't take off the price if you get something taken off?

Rarely, some restaurants take off the price, but not all of them-what I do is ask them to "subsitute" one thing for another, like "no tomatoes, extra cheese"

Because it still takes the extra 'manpower' to serve you something that is not the standard item as they serve it.
