I'd like to say its an effort to minimize her, due to her female gender, and maybe that's how it was meant to be, but really, its just easier to say Hillary, or Bill than say Clinton and figure out which one.
His name is Barak Obama, so calling him Obama is using his last name.
Rudy seems to prefer that possibly because Giuliani is one of the few extremely ethnic names to be running for president at any time.

I think it's because we have grown to know Bill and Hillary over the years until they have become a household name. For good or bad they are much like family. Bills affair seemed to make him more human like your neighbor or your uncle. Hillary had to face the facts of his infidelity like many other women have in front of the whole community or town only she had to face a nation. I still think they stink though.

it's more of a differentiation between Hillary and Bill, since Bill has been president already and Hillary is in the running. Though I know what you mean.

I hear a lot of people using Giuliani's first name though.

Who are you talking about Obama, Hillary or Rudy? If you let us know which one of the three that people call by their first name who is the only one that people call by their first name, it will be easier to answer your question.

Using Hillary Clinton's first name has become common in order to differentiate between her & her husband.
