That depends a lot of where you are in school and what your class is like. If you are at a highly competitive school, which tends to funnel many of their students into Ivies and other top schools, you can get in if you are not one of the best students in your class, but if most of your classmates never go on to college, then yes, you need to be at the top of your class to get in.

it helps. but you should look at their requirements and statistics. because colleges don't just care if you're the number one student at your school. they care if you're well rounded (sports, clubs, other extracurricular activities) and if you are consistent (did you do something all four years, did you join a club for only one year and did that with all of your other clubs).
there's also the personal essays. my teacher have been talking about how the essays really matter now because univerisites are looking at them more.
