Ruth (Boston).
Aaron (Milwaukee).
Mays (New York).
Hmm... gonna have to research the fourth.
(Ron Santo, if the new VC gets its head out of its backend in 2009.)
-----
Edit:
Ernie Lombardi, 1931 Brooklyn Dodgers and 1947 New York Giants.
Freddie Lindstrom, 1924 NY Giants and 1936 Brooklyn Dodgers.
Jim Bottomley, 1922 St. Louis Cardinals and 1937 St. Louis Browns.
Sam Thompson, 1885 Detroit Wolverines (National League) and 1906 Detroit Tigers.
George Kelly, 1915 NY Giants and 1932 Brooklyn Dodgers.
Yogi Berra, 1946 NY Yankees and 1965 NY Mets (this one usually surprises the peanuts out of folks, but yes, Yogi appeared in four games for the Mets two years after he'd retired from the Yanks).
Waite Hoyt, 1918 Giants and 1938 Dodgers (obviously the popular method).
James "Pud" Galvin, 1875 St. Louis Brown Stockings (American Association) and 1892 St. Louis Cardinals.
Dizzy Dean, 1930 St. Louis Cardinals and 1947 St. Louis Browns.
Harry Wright, 1871 Boston Red Stockings (National Association) and 1877 Boston Red Caps (NL, now the Braves). But he wasn't inducted for his playing career.
Jimmie Foxx, 1925 Philadelphia Athletics and 1945 Philadelphia Phillies.
Rogers Hornsby, 1915 St. Louis Cardinals and 1937 St. Louis Browns.
Cap Anson started with the 1871 Rockford Forest Citys and finished with the 1897 Chicago Colts (now Cubs), so he was geographically very close.
Nap Lajoie, 1896 Philadelphia Phillies and 1916 Philadelphia Athletics.
That's all of 'em, I do think. Just to complete:
Ruth -- 1914 Boston Red Sox and 1935 Boston Braves.
Aaron -- 1954 Milwaukee Braves and 1976 Milwaukee Brewers.
Mays -- 1951 NY Giants and 1973 NY Mets.
There's more than four. Aside from the Ruth-Aaron-Mays troika, whom are all widely-known names, the most popular fourth would likely be Berra, followed by Foxx, Hornsby, and Dean. The other guys, worthy HOFers all, are lesser known names from this far remove in time.
Fun question. I was surprised there was no player who did it with Chicago, but perhaps Santo will change that. (C'mon, VC.)

Babe Ruth was certainly one of them, starting with the Boston Red Sox and finishing with the Boston Braves.
Willie Mays started with the New York Giants and finished with the New York Mets.
Yogi Berra started with the Yankees and finished with the Mets.
Certainly candidates would have played with Chicago, Milwaukee, an NY team (if you don't consider the individual cities inside NY) or Los Angeles. Also, I guess Seattle could be a possibility (pilots and mariners)
