Holy crap. A whole lot of misinformation. For the woman who said it was easy for women, it's NOT. I recently did it and it so far has been a lot of work.
Here's some ACCURATE information. To change your maiden name to your married name do this:
1. Order a CERTIFIED copy of your marriage certificate or go to your county recorder's office.
2. Take copy of certificate to Social Security, fill out the proper forms, have your old driver's license, and sign paperwork to verify your new name.
3. Take your information from the SS office (you'll get a receipt and slip of paper with your number (this wont change) and new name) and go to the DMV office. Once you're called, show your certificate, surrender your old license, show your proof from SS, fill out the necessary forms, sign all paperwork, and when instructed, take your picture. Depending on your state you'll either get a new license right away or in a few weeks.
4. After DMV and SS are secured, take care of your banks, utilities, e-mails, catalogs, credit cards, car insurance, health insurance, dental insurance, etc.
This is all daunting, challenging, and takes up time and effort but you'll get through it.
Good luck and congrats!
EDIT:
I got a thumbs down for giving ACCURATE information? Good grief. I must have pissed off one of the ignorant ones.

there is not any justification (or absolution, it fairly is honestly what you look to need) for having an affair. you have suggested that she delivered persons into your place and mattress room, and that there is a one-sided open marriage (and subsequently, none in any respect). you have allowed this trend to proceed, to the quantity that the two: a) you have not issued an ultimatum (end doing those issues or this is over), or b) you have not accompanied by way of and she or he does not take you heavily. So: Step one million: determine you have a great communicate and that neither of you leaves till you the two understand the place one yet another stands. Step 2: If she isn't keen to alter or regulate what she's doing, it fairly is obviously hurting you, then this is already over, and a divorce is in basic terms a criminal attractiveness of that actuality. i don't understand your particular info (little ones? etc.) yet based off what you offered, those are your thoughts.

Should have done it when you first got married.
And this may just be a state to state thing, but I believe that you only have a certain amount of time and you don't get to switch back and forth.
You will need your birth certificate and your marriage license. Check out your local dmv, sss, and county court house websites for more info.
I know in GA, you only have a certain amount of time after a marriage or divorce to switch your name on your DL and SSC. Doing so after that period requires you to go back to court and petition to have your name changed. And now they are saying that if your ssc and dl don't match that they can right you a ticket.

Do you have a marriage certificate yet? If you haven't, order one. Then you just use that to go and do the name change. Women don't need to do anything special to use their husband's name. You just have to have th marriage certificate to prove you're married. You can also switch back to your maiden name whenever you feel like it.
Edit: I should point out this only applies to you if you're Canadian.

Actually, I think all you have to do is go to the Social Security office and get your number in your married name, and then start changing your name with new applications from wherever you have a credit card, your job, etc. etc. you get the idea?
