Likely I would be the one reading :) I love reading too, and it's unlikely I would bother anyone who was reading a book, but if I had to say or ask something, I'd probably say 'sorry for interrupting' and be quite understanding that as soon as I had my question answered, you'd want to go back to your book!

no you shouldn't be interrupted while you are reading.
So I wouldn't be offended. I am guilty of interrupting people while reading usually it's because I am bored.
Unless you are somewhere that you were invited, such as having dinner with someone(i have actually seen a person do this)

No, not offended at all. I should be happy that you chatted with me at all. I really hate to be interrupted when I am reading.

No. People do it all the time, and if you're the least bit observant, you'll see that they're interested in reading the book and leave them alone.
