There is only two ways you can commit the unforgivable sin in Christian theology:
1. Witness a miracle by Christ and attribute it to Satan (this is no longer possible). Testifying against Christ also fits in this category (again this is no longer possible). These amount to acting against Christ.
2. To completely and utterly deny the Holy Spirit to point where God accepts the fact you are beyond saving. When this happens, the Holy Spirit abandons you and salvation no longer become possible.
#2 is actually harder to commit then one might think. Because it's not a matter of what you say, but what you believe in your heart. It must be absolute, you have denied God/Christ with every fabric of your being.
If one's conscience is bothering them for the sins they have committed, the very fact that they are bothered by their sinfulness shows that the Holy Spirit has not abandoned them and nor has God given them over to a depraved mind.
If someone denies Christ up until judgment, this can also be construed as part of the unforgivable sin.

There is only one unforgivable sin and that is blaspheming against the Holy Spirit. And in answer to your question, yes they can believe in Jesus and be Baptized and still commit this sin. Believing and being baptized doesn't get one into Heaven. You also must have a personal relationship with Christ.

Not only can they, but ONLY those with a correct understanding of Jesus can commit the unforgivable sin.
In order to blaspheme the Holy Spirit, one must attribute the work of the Holy Spirit to Satan and his forces. The verses where Jesus describes the unforgivable sin follows just after he is confronted by the Pharisees who SEE HIM perform a miracle by healing a man. They then turn and say that his power is from Satan.
What is critical to understand is that the pharisees knew enough about Jesus to make a clear decision about who he was. In other words, they knew he was working under the power of God himself. But to now publicly accept his claims would mean they would forfeit their considerable leverage over the Jews of the day, and even to a degree their Roman "lords."
So, back to your question, those that simply disregard Jesus, The Spirit and God are NOT blaspheming the spirit. This would be more accurately labeled "blindness" or "spiritual ignorance." But to deny their existence does not qualify as an unforgivable sin. At one point we all did this through our actions, position of our heart and life choices. Only after being drawn to God by his will did we repent from this and accept Jesus. Hence we are forgiven. Denial is "a type" of blasphemy, and one we have all been guilty of. But it is not the blasphemy that Jesus spoke of.
A really challenging topic to look into is if salvation can be lost. There are powerful arguments in both camps of this debate. Specifically, check out the scriptures in Mathew where Jesus says "depart from me, I never knew you." This issue has a parallel to the above mentioned topic in that only those with a correct understanding of Jesus can commit the blasphemy he describes. Yet, this would indicate that not all that HAVE a correct understanding of Jesus ARE saved. (Faith versus works) (Pharisees being a great example. But they were not saved so far as we know.)

Yes. King David almost committed the unpardonable sin. If GOD did NOT send Nathan to tell David of his sins, he might have been lost. When David heard the prophet, he feel on his face for 7 days lamenting and repenting.
Read 2 Samuel 12. Also, read the prayer of repentance in Psalms 51.

No, he's not supposed to, and if he did than he's not Christian anymore, he's a sinner.
But through Jesus Christ, sinners can always repent. They can admit their mistakes through prayer, get saved, by accepting Jesus Christ as his/her saviour, and by getting baptized.
If you sin again, than I guess you have to start all over again.

Only can be commited when one is standing in front of Anti-Christ, testifing against him. If one doesn't allow the Holy Spirit to speak thru him..thats the sin.
