Depends on the nationality of your friends passport and how long your friend plans to spend in the U.S. and for what purpose.

Edit:

In regards to your additional, it all depends on how you both intend to enter the U.S.

If you are entering the states by air then you both will need a passport and nothing else.

If your friend is entering via land or sea then your friend does not require a passport but you must have Canadian citizenship documents such as a certified copy (not a photocopy or facsimile) of a Canadian birth certificate, this must be accompanied by a government issued photo-ID such as a drivers license or military ID etc to gain re-entry back Canada.

No visa is required for Canadian citizens or permanent residents of Canada to enter the U.S.

It sounds like you and her are minors traveling with you parents. She should get a letter of consent signed by her parents that details the specifics of the trip and permission to travel with your parents. Sample is in the attached link.

If it is just you and her traveling as minors without your parents, you both need one.
