something i did when my son was small and needed splinter out of feet & hands was first placed my body between area hurt and what he could see, and placed a ice pack on it for a few mins, deadens pain. then my husband use to hold his hand and talk to him about anything just to keep his mind off what i was doing.
then i would pull splinter out, if possible squezze area alittle to get puss out and then wash area with anti-bacteria soap like dial or lever 2000 and then triple antibotic ointment and a bandaid

I don't know how old your brother is...but if he's little, calm him down and re-assure him that they won't cut it off. The best way I've found is to sterilize a needle and go under the splinter and try to lift and pull at the same time. Don't let him see the needle because it may scare him. It doesn't hurt much and I have always had luck getting them out. Tell him also that once you get it out it will quit hurting all together. If it's infected and you go underneath it that should help to loosen it and push it out. I have small children and they prefer me to take out splinters over their dad! Ask him what's better...to leave it in there and let it hurt or get it out and make it stop hurting. Be calm yourself and that helps to calm him, too. Little kids tend to freak out more than older people do. Once the splinter is out put some peroxide on it. If you are calm and get it out you'll be his hero.

Is it red and gooey with white stuff? If it is, then press gently on either side of the wound. Usually the body makes a little bag around the foreign body to help it propel out of the body.
Tell your brother that it has to be green or black and really, rotton stinky before they cut them off.
Let Dad try to get at it, but don't let brother look. If you have bactine or neosporin with pain meds in them--rub his toe down with that, wait a couple of minutes, then tell him it won't hurt because of the medicine. Pull it out with clean tweezers, open it a little with a clean needle if you must, and if you can't, take him to the dr. in the morning.
Check the inside of his ankle and see if there's a red streak going from ankle bone 6 inches or more toward the knee. This requires the ER.

Honestly, when my kids have gotten stickers and things in their feet I tell them that it only hurts to leave it in and then I straddle their legs facing their feet (so they can't see what I am going to do). Then I pull out the tweezers that I have palmed and the bottle of peroxide that I let then see. I tell them to let me pour a little on the toe to see what's there and then pour it on and while it is bubbling, I pull out the splinter out quickly. A lot of times, they don't even realize it is gone and then I show it to them and they get to tell me how silly it was to act the way they did.

if it's infected, go see a doctor, if you delay more, the infection will get worst, and the doctor really have to cut his toe off.
Reassure your brother, talk to him, and let him know that removal of splinter is very fast.
