Originally comes from college football, in the old days when there was little specialization. A "triple threat" could run, catch, and pass (or perhaps kick). Check out Red Grange or Knute Rockne.

Whenever I've heard it, it seems to refer to something that has three qualities; usually desirable qualities that will give an advantage over competitors. Such as -
A movie with three big-name stars
A martial-arts fighter with proficiency in three styles
