These transmissions need proper adjustment of the throttle valve(a cable attach to the throttle and move a pressure valve in the valve body)to control working pressure,many amateurs or so-called shade tree mechanics are not familiar with this.It is my best guess,hope this helps.

Hard to answer. If, you live in a Hill/Mountain area this may be the problem. People need to not use the OD it hard on the transmission. The OD is for when we on the freeway and moving on down the road. I not remember off hand, but, I thinking the OD use a .7:1 ratio so it put a stress on it shifting down all the time in hills. My Chevy Truck, I pull a 32' 5th wheel camper, it have that 700-R4 OD. When I bought the truck my friend that rebuilds transmissions told me; "I get a lot of those". I asked, what can I do? He said; "When you pulling the 5th wheel, do not use the OD"! I have done that and it a 1990 been pulling that 5th wheel for 12 years and it still going. I changed the Fluid and Filter once when I bought it in 1994.
But, if you not live where there a lot of hills or mountains, then I just wasted my story and not know. :-p

To add to Carmen Nguyen's excellent answer, those specific transmissions were famous for burning out the direct clutches and overdrive bands in their time. The hollow input shafts, exclusive to that particular transmission, were also another weak link. For this reason, when Ford introduced the AOD-E/4R70W transmission (the AOD's replacement), they changed the internal setup to use a "stub shaft" between the input and output shafts, while using a conventional lock-up type torque converter, which replaced the hollow and solid input shaft setup on the AOD, which had the solid shaft "locked" to the engine crankshaft, meaning this shaft would be spinning when the engine is running, regardless of manual lever selection.

I'm with Carman. My guess would be that you need to offer free TV cable installation/adjustment service with your transmissions.
If the TV cable gets unhooked they won't shift, if I remember right.
