how do i prepare a professional salary history document along with my resume?

i am applying for a job that is asking me to send my salary history along with my resume, what does a professional salary history document look like? i have never seen one of these before

Don't over-complicate it. I never provide this, even if they ask. If they don't like it, I go elsewhere. Providing your salary can only hurt you. If they really want it, they can do a background check.
