If you are within the US already, it is easy. Make photocopies of your passport, copies of your I-94 and visa. Provide a letter explaining why you want to stay in US. Submit statement from the bank that would show about $ 5000 to support yourself during your stay. Fill in an application for Extension or change of non-immigrant status. Attach a check or money order with the required fee. Seal everything in the envelope and send it to USCIS.

You need to go to the Embassy of the country you want to visit and apply for a visa. You'll need your passport and whatever other documents they require.

It would depend on the country. Are you immigrating or emigrating? (Coming in or going out). You need to give more specifics: you are an american citizen traveling to what country when?
