According to the Official website of the Italian Foreign Ministry, if you live in Jamaica you will have to apply for a visa at the Italian Embassy in the Dominican Republic.

this is the form that all consulates will give you, your daugther has to apply from Jamaica, even if you filled out the form, if she is too young? then her father perhaps?
