My cavities never hurt until they get down into the root. I've had molars where half the tooth was gone and it still would not hurt for years. Then eventually it gets infected and has to be removed. And yeah, it is possible to need a root canal even if there is no pain. A dental X-ray is the only way to determine this.

Not necessarily, if caries stays in the enamel usually it is painless. cavities do hurt if it reaches the dentin and pulp (nerve and blood vessels). if the tooth is dead (from trauma..e.g. sports accident) without abscess, it is not painful. the dentist will do root canal treatment then.

MOST CAVITIES WHENN FIRST FOUND BY DENTISTS DON'T HURT.
IF YOU'VE LET THE CAVITY GROW SO BIG AND NOW A ROOT CANAL IS NEEDED, THIS WOULD HURT.

They don't 'always' hurt.
But its best to get a filling before it DOES get huge and then you'd have to get a root canal!
