impeccably groomed comes in 10 min. late with no makeup and uncombed hair would you chart any of these observations in her chart?

If I worked in a Dr. office I probably would. But I'd ask her if she was in a hurry today & such first. Give people the benefit of the doubt. She may not work on that day & thus not be all prettied up. I don't even wear deodorant if I'm in for x-rays or some other test or a physical. I dry my hair out naturally before combing it. She might be this way as well.

No, these type of things are not generally caused by a medical condition; so there would be no reason to notate it in her chart.

If the doctor mentioned it after the patient was finished with the office visit, it could be notated then as it may be considered to be part of the general diagnosis.

Yes .... things change in ones life but someone needs to notice and make notes. One day it might matter.
