That happened to me as well.
I dialed 911 on the cell phone and gave them the plate number. I had an expression on my face that made it look like I was having fun and not panicing because I didn't know if it would alert the person that I was calling for help. The police eventually pulled in directly behind them and arrested the girl.
It so happens that I had a prior complaint at the attorney generals office. I dont know if yours is that serious.

Drive to a heavily populated, well lit area... if you can - find a police officer or other authority figure. Write down a description of the vehicle, including license number... if you've got a cell phone - call 9-1-1... use the camera in your phone to take a pic of the driver, the car, & the license plate.

Do NOT confront the person (or force a confrontation by slowing down / slamming on your brakes / etc.).

slow down and pretend not to see what is going on around you and try to get behind the person who harasses you and get the plate number,report to police. Breaking suddenly and boxing them in only escalate the situation.

In addition to Michael's suggestions, drive to the nearest police station or fire department; if none of those are nearby, the busiest parking lot available.

if you have a cell phone set them up by calling 911 and lead them right to a police station. If they realize what your doing and speed away type their plate number in your phone and press charges for harrashment and or stalking. But you better not be the one causing the drama.

Lock your doors, drive near a police station to scare them off or get your cell phone out and pretend to call 911. I usually slam my breaks to let them know to get off my rear. Write down their license plate number and report them to police!

this is where a bike comes in very handy. a typical cager wont be able to match a bike in how quick it can skate across 3 lanes of traffic and take an off ramp. u could also sucker him into tailing u, get on a street that u'd know that would wind up having heavy congestion, then do something that is guarenteed to ditch the b itch in spades.....split the lane and thread the needle. no cager will be able to match that.

drive to a police station - they won't mess with you there - slamming on brakes, driving erratically etc... will just escalate the situation making it more dangerous.

If you carry a gun, you could shoot them. Otherwise might I suggest really pissing them off by boxing them in between other cars.
