The treadmill can burn 16-20 calories per minute of exercise during your workout. Of course only advanced fitness levels can achieve that (not sure what your fitness level is).
Some tips: Don't go crazy with the incline (as that can lead to tight calves), but if you are able to run at more than 10mph, use a 1% incline to help offset the need for speed.
If you have bad knees the elliptical is probably best. A stationary bike will allow you to work hard with less risk of injury. It really is up to you and your particular needs but it seems most of them do a pretty good job at fat burning.

Your legs, provided you are in good health and have no problems with your joints. Can't go outdoors? Use the stairs, or a step up box. Boring, but so are the machines. Ninety-nine percent of the Cardio-machines purchased for home use end up being used as clothes-racks.
Whatever you do, try them out first. Some of them are nothing more than motion machines that you can use for an hour and not even break a sweat. The harder you have to work, the better cardio workout you get.
The rowing machine is a good workout. The treadmill is good if you can adjust the angle of the walking board, the steeper the angle the better.
That elliptical walker with the levers for your arms is a useless motion machine. You are using both the power of your legs and arms so you aren't getting much of workout at all.
Swimming at the Y is good. Power walking is very good (and cheap) and uses all the major muscles.

What do you mean grappling. Do you go to a gym or have an instructor? He should be able to tell you exactly what to do. I would stay clear of lifting every single day as it's not good for you. Do the three main body parts once a week and you'll be fine, providing that you actually work hard and do an intense workout each time. Your weight lifting routine should look like this (or similar): Monday: Chest, Tuesday: Rest/Calisthenics, Wednesday: Back, Thursday: Rest,Calisthenics, Friday: Legs, Saturday: Rest/Calisthenics, Sunday: Rest/Calisthenics The idea is that you need at least one day in between each weight training session. Feel free to do push-ups, pull-ups, and abs whenever you can, as long as they don't interfere with your weights. Calisthenics improve mostly muscle stamina and give you some small strength gains while weights are mostly muscle mass and strength. Cardio should be done everyday. I don't know about doing it before breakfast because you could always eat early and start your cardio after an hour or so to digest. More carbs that way for energy. Martial arts training everyday is fine, provided you don't strain your muscles in a bad way. You don't need to do cardio twice a day unless your legs aren't too sore.

Everyone can use the traditional treadmill. Machines at the lower price range are affordable for most families.
