The facility manager, if no action call the fire marshall.

Always go first to your immediate supervisor. If that fails, your company may have a safety committee to whom you can report the problem. Going to OSHA is a last resort. If you do go to OSHA you may well be fired later on for some "mistake" or other.

make sure you put it in writing giving they copy to the person or persons keep the original. first state the area, then how is it unsafe. Then state what you would like to see done to accomplish a safe situation. The one to contact is the supervisor. Or your humane resource person. good luck

Most every company has one now a days.

managment first and if they dont fix it threaten to call osha that usually gets it done, but if not, call osha and they'll send someone to check it out
