Question: "What causes the most suffering to man?"
Solitude. Man is a gregarious species, and relies on the interaction of others to maintain and increase his capacities. Put a man in solitude, and just as you see an old man or woman decay faster in a care center than they would if they were around the movements of family life, so are the living attributes possessed by a man slowly lost.
Spiritually speaking, I believe sin (what one would typically say is what causes the most suffering to man, ultimately) leads to solitude and darkness of the intellect, of emotions, and basically of the spirit of man.

Death of someone you love. That is why God so badly wants us to return to Him, as we are all his children. In Another words suffering of the soul is much worse than the body. That is why Hell is often compared to a burning pit because how else do you compare the loss of your soul when you know that you will never live with those you love that would be hell for eternity.
