I think this question is fairly straightforward. In addition what are the signs one can look for in any case a woman is in fact faking the big "o"?

Partly because faking one sometimes causes a real one to happen.

Don't worry about it; detective work during coitus causes anxiety.

Well....
If a man REALLY is paying attention, he should be able to tell (I think I could, if I were a man).

Basically, whatever she says, moans shudders, twitches, etc. can all be a big smoke screen. The orgasm itself is a series of muscles spasms, lasting anywhere from a few seconds to-- if you're really good-- a few minutes. Not that many women actually have orgasms during intercourse, but if your woman does, you should feel something rapidly squeezing your penis (sorry for being so explicit) and she will probably be in a state of suspended animation while it is occurring. LOL
Personally, I don't like to be touched if I'm having one. (She might push your hands away and say "stop!")

More likely, though, she'll reach orgasm by some other method, like manual or oral stimulation. Again, while it's occurring, she may want you to halt for a moment.

But all that "oooohh! Yes! Yes!" crap is for the movies. Don't fall for that.

Depends on how long he has been with her and how well he know her intimately. I honestly wouldn't be able to tell if it was the first or 2nd time because all women express it differently. Some are very quiet while other shout out and thrash about. The bigger question is if she was really into the guy why fake it? If she isn't getting there then the only way to fix it is to communicate and try to find a way to get there.

You want the truth? All right heres the truth. As she is going through the motions look at her neck and chest they should be almost glowing red. It puts one in mind of a skin rash that covers the whole area and this is the only thing that a girl can't fake. You can tell if you pay attention. When a woman gets hers, her groin will spasm. The better the orgasm, the harder and faster the spasms. They can be faked, but once you get a feel for them, you'll be able to tell the difference. Look up the kegle muscle as it's the one doing all the moving :).

You probably won't know. Every woman experiences them differently, and a lot of times they're different for the same woman.

But it's not always a bad thing if she is. . . she may have had a wonderful time, but she's just ready for it to be finished. Some women have a hard time getting to orgasm, and everyone needs to sleep!
