Attend the jury duty and from there you can formulate the questions you want to ask and how you can participate in the proceedings.

Call The Clerk of Courts Office to see when to report. If you don't show up the judge can issue a "Bench Warrant" for your arrest.

Blow it off and go fishing like I would after making a visit to my doctor for an undisclosed medical reason.

I wouldn't worry about it. When you get there, tell them you misplaced it. I'm sure it happens all the time.
