Well, I'm sure some are out there working with no coverage. I don't know of any LAWS requiring malpractice insurance, however, no hospital is going to let the doc in the door without them having malpractice in place.
Pricing depends on A LOT of factors - like location, limits, prior claims history, how many years they've been in practice (especially for the retro date), what kind of office procedures they do, if they have hospital priveleges, how many hours they work, etc.
A gp is going to have all the expenses of ANY small business owner - other insurance for the office, property & liability, workers comp, health insurance for employees, utilities, supplies, equipment, etc.

Yes, GP's have to pay malpractice insurance. Usually they are the specialists that pay the least, may be because their risk of being sued is less than others.
Expenses in addition to malpractice include office space, nurse, secretary, memberships, licenses, light, gas, etc.

All doctors need malpractice insurance. Cost depends on type of practice and location.
Renting and equipping an office is very expensive.
