I am assuming this is regarding a non-immigrant visa for the United States.

Permission to enter and/or remain in the U.S. and extensions of stay in this country are granted by the Department of Homeland Security (DHS). If you came to the U.S. on a non-immigrant visa, and you want to extend your stay in the U.S., then you must ask for permission from DHS’s United States Citizenship and Immigration Services (USCIS) before your authorized stay expires. If you have a date on your Form I-94, you should apply for your extension of stay with USCIS well in advance of its expiration.

Do you mean extention of stay? I take it that you hold multiple entry VISA. You must apply to the appropriate office citing an unavoidable cicumstance and if they're convinced they may give you permit to stay for a certain period after expiry of your legal stay in the US. But this will be a negative point when you apply after expiry of your Visa. Think about it. The alternate to this is you must leave the US, even for a day and re-enter again to stay another 180 days.

Visa can be extended if there is a valid reason.
