Either his mother, his gay "other" dad, his adoptive/step father (the man who died was the biological one), or his biological father (the man who died was the adoptive/step one).
This riddle was designed to trick people who traditionally think of a doctor as a man and would be confused by the fact that the father apparently died already, thus not thinking a woman (the mother) could be the doctor. Well, times have changed and now their are boys with two gay fathers, adoptive fathers, stepfathers, etc.
