There's no really a difference. The "me" is a kind of pronoun that can be attached to the verb and in Spanish we use them all the time. So it'd be better if you say: "Tengo que salirme temprano" (I have to leave early)
You could also say "Me tengo que salir temprano" and is perfectly right. The 2 forms are better than just "Tengo que salir temprano".
Just be careful and don't use them twice: "Me tengo que salirme temprano" because you will be repeating that it's you and it's a mistake.

both salir and salirse can mean to leave
in this case the pronomial form doesn't change the meaning - it seems to just make it stronger.
Perhaps the second could be "I have to escape early"
