You can certainly clean near the opening of the vent with your vacuum cleaner, but realistically, you won't be able to pick up much dust and dirt more than an inch or so beyond the end of the vacuum nozzle.
Businesses that service ductwork use special gear to clean the whole length of the duct... and all around, the duct... dust doesnt just build up on the bottom, it will coat the entire inner surface. And to help remove mold, most of these companies will use a steam attachment, to kill the mold, and any bacteria that might be present.

Yes you can but the people that specialize in duct cleaning have advanced tools that can reach down and clean the entire duct cavities depending upon the distance, you can do this every so many years and get out the majority of the debris.

It is not important what is in the duct it is important what passes through the filter and contaminates the air in the house. Duct cleaning kicks everything up and puts it in your living space. Buy pleated filters or have an EAC installed. Electronic air cleaner is 99.9 % effecient. It kills mols spores, dust, and dust mites, pollin and particals down to 500 microns.

you need duct cleaning tools like a chimney sweeper needs different tools than a vacuum cleaner.
